How to Order
You are just three easy steps away from ordering the Cleo® 90 infusion set, the single-use disposable infusion set from Smiths Medical MD, Inc.
The Cleo® 90 infusion set is an all-in-one system that lets you control the insertion process. It doesn’t require a separate inserter, so it’s ready to go when you are.
To place your order:
1. Fill out a Prescription Request for the Cleo® 90 infusion set (if you don’t currently order supplies from Smiths Medical).

2. Complete the form and take it to your physician for signature.
3. Fax the completed form to Smiths Medical Customer Service at 800-209-2145.
If you do not have insurance information already on file with Smiths Medical, please complete the form below and fax it along with your prescription form to Smiths Medical Customer Service at 800-209-2145.

· Once we receive your prescription form and/or insurance information, we will contact you about placing your order for Cleo® 90 infusion sets.
· We will verify and confirm your coverage with your insurance company. We will contact you when we have received all of the required information and process your order.
· Your order will be shipped to your address on file.
